To report a claim situation, board complaint, or professional incident, or to collect other policy benefits, click on “Report a Claim”
Change your personal information like address, phone, fax, email address etc. by clicking on “Change Your Address Online.”
All other requests to change your policy must be approved by the underwriter and must be made by written request. Use the printable documents found under the “Make Changes to Your Policy” link above.
